Community Referrals

Case Management services are designed to help students get connected to community referrals. Clients whose needs fall outside the scope of services or scheduling availability of the Student Counseling Center (SCC) are referred to community resources. Such referrals might occur immediately following the initial appointment or after some treatment and a further need assessment. The Student Counseling Center Case Manager provides community referral options to meet a client’s needs best and follows up to offer additional assistance finding resources.

When a community referral is the most appropriate treatment option for clients, they will be provided several community referral options. The Case Manager will work closely with them to ensure they are connected to a community provider. To the best of the Case Manager’s ability, these options will address client circumstances regarding health insurance, finances, and transportation.

Examples of community referrals the Case Manager can assist with:

  1. Community referrals for long-term, intensive support and specialized care,
  2. Medication management,
  3. Basic needs include food insecurities, housing, transportation, clothing/hygiene, and financial assistance,
  4. Other community resources include but are not limited to support groups, violence prevention resources, psychological testing referrals, outpatient treatment services, and court-ordered/mandated assessments and treatment.
